Your gums not only help hold your teeth securely in place, they also help protect them. They're also part of your smile — when healthy and proportionally sized, they provide a beautiful frame for your teeth.

But if they become weakened by periodontal (gum) disease, they can detach and begin to shrink back or recede from the teeth. Not only will your smile be less attractive, but you could eventually lose teeth and some of the underlying bone.

Treating gum recession begins with treating the gum disease that caused it. The primary goal is to remove the source of the disease, a thin film of food particles and bacteria called dental plaque, from all tooth and gum surfaces. This may take several sessions, but eventually the infected gums should begin showing signs of health.

If the recession has been severe, however, we may have to assist their healing by grafting donor tissue to the recession site. Not only does this provide cover for exposed tooth surfaces, it also provides a “scaffold” for new tissue growth to build upon.

There are two basic surgical approaches to gum tissue grafting. One is called free gingival grafting in which we first completely remove a thin layer of surface skin from the mouth palate or a similar site with tissue similar to the gums. We then attach the removed skin to the recession site where it and the donor site will usually heal in a predictable manner.

The other approach is called connective tissue grafting and is often necessary when there's extensive root exposure. The tissue is usually taken from below the surface of the patient's own palate and then attached to the recession site where it's covered by the surrounding adjacent tissue. Called a pedicle or flap, this covering of tissue provides a blood supply that will continue to nourish the graft.

Both of these techniques, but especially the latter, require extensive training and micro-surgical experience. The end result is nothing less than stunning — the tissues further rejuvenate and re-attach to the teeth. The teeth regain their protection and health — and you'll regain your beautiful smile.

If you've suddenly noticed your smile looking more “toothy,” you may have a problem with your gums. They may have lost their normal attachment to your tooth and begun to shrink back — or recede.

Millions of people have some form of gum recession. The most common cause is periodontal (gum) disease, but it's not the only one. You may be more susceptible to gum recession because of heredity — you have thin gum tissues passed down to you from your parents. You may also be brushing too hard and too often and have damaged your gums.

Healthy gums play an important role in dental health. The crown, the tooth's visible part, is covered with a hard, protective shell called enamel. As the enamel ends near where the root begins, the gums take over, forming a tight band around the tooth to protect the roots from bacteria and acid.

Receding gums expose these areas of the tooth meant to be covered. This can lead to another tell-tale sign — tooth sensitivity. You begin to notice pain and discomfort while you consume hot or cold foods. And because it leaves your teeth and gums looking much less attractive, it can affect your confidence to smile.

Fortunately, though, we can help restore receded gums. If you have gum disease, it's imperative we treat it as early as possible. We do this by removing plaque, a thin film of bacteria and food particles that triggers the infection. We use special techniques and hand instruments to remove plaque and calculus (hardened plaque deposits) from all tooth surfaces including along the roots.

Gum disease treatment can help stop and even reverse gum recession. In some cases, though, the recession may have advanced too far. If so, we may need to consider surgically grafting donor tissue to the recessed areas. Depending on the site and extent of recession, this can be a very involved procedure requiring microscopic techniques.

The best approach, though, is to take care of your gums now. Daily brushing and flossing removes harmful plaque; regular dental visits take cleaning a step further and also give us an opportunity to detect disease early. By looking out for your gums now you might be able to avoid gum recession in the future.

Your mouth is a lot like the Wild West — home to millions of bacteria and other microbes, some of which are definitely not “the good guys.” But your teeth are well-protected from these hostile forces and their acidic waste products: with enamel shielding the visible part of your tooth, your gums protect the parts you can’t see.

As effective as they are, though, your gums aren’t invincible: their greatest threat is periodontal (gum) disease. This bacterial infection arises from plaque, a thin film of bacteria and food particles accumulating on teeth due to inadequate brushing and flossing.

The infected tissues soon become inflamed (red and swollen), a natural defensive response from the immune system. The longer they’re inflamed, however, the more likely they’ll begin detaching from the teeth. The gums may eventually shrink back or recede from the teeth, often causing them to appear “longer” because more of the tooth is now exposed to view.

Gum recession doesn’t bode well for your teeth’s survival: the exposed tooth and underlying bone can become even more susceptible to infection and damage. In the end, you could lose your tooth and portions of the supporting bone.

Treatment depends on the severity of the gum recession. In mild to moderate cases, we may only need to perform the standard gum disease treatment of removing plaque and calculus from all gum and tooth surfaces (including below the gum line) with special instruments. This helps reduce the infection and allow the gums to heal and re-establish attachment with the tooth. In more advanced cases, though, the recession may be so extensive we’ll need to graft donor tissue to the area using one of a variety of surgical techniques.

Although the right treatment plan can help restore your gum health, there’s another approach that’s even better — preventing gum disease in the first place. You can reduce your disease risk by practicing daily brushing and flossing and visiting your dentist regularly or when you see symptoms like gum swelling or bleeding. Taking care of your gums won’t just save your smile — it might also save your teeth.

Did you know that millions of Americans have some degree of gum recession? Are you one of them?

Gum recession is the loss of the pink gum tissue that surrounds your teeth and can lead to exposure of the root surface of your teeth. In addition to the obvious aesthetic issues, recession can also result in tooth loss in very severe cases.

So, what causes gum recession? Well, first of all, if you are genetically predisposed to having thin gum tissues, your gums will be more prone to receding than those with thick tissues. However, other factors include ineffective oral hygiene, excessive brushing and mal-positioned teeth. In addition, poor fitting appliances, such as partial dentures can also cause gum recession.

If you think you are suffering from gum recession, you should make an appointment with us immediately, so that we can perform a thorough examination to accurately diagnose your condition. We'll look at your teeth and their position within the supporting bone and surrounding gum tissue. Depending upon our diagnosis, we may recommend a technique known as gum or soft tissue grafting, which allows us to regenerate lost or damaged gum tissue. Grafting is the surgical manipulation of tissue, taking it from one site and moving it to another, so that it can attach and grow.

There are two basic gum tissue grafting techniques, the free gingival graft and the connective tissue graft. Here is a description of each:

  • Free Gingival Grafting. With this technique, we remove a thin layer of tissue from the roof of your mouth or any other site where the tissues are identical to gum tissue (the donor). We then shape and transplant it to the recipient site to create new gum tissue. Both donor and recipient sites heal within two to three weeks.
  • Connective Tissue Grafting. This technique is used to cover exposed roots in the treatment of gum recession. It involves more microsurgical maneuvers to prepare both the donor and recipient sites. We take donor tissue from beneath the surface of the roof of your mouth and then cover it with the gum tissue surrounding the exposed root. Another alternative is to use processed tissue rather than your own tissue as a donor material.

While hygiene and regular dental care go a long way to reduce your risk of oral disease and disorders, you’re still subject to your heredity. Everything from tooth alignment to the shape of your jaws is determined by your genes.

So is the biological structure of your gum tissue. Aside from minute variations, gum tissue structure falls into two broad categories — “thin” or “thick,” which refer to the actual thickness of the tissue and the underlying bone. The tooth’s appearance is the best indicator of which type you may have: those with more triangular-shaped tooth (often called scalloped) have thin gum tissue; a person with a squarer appearance (flat) has thick gum tissue. People of Asian descent tend to have thin/scalloped tissue while those with European or African heritage tend to have thick/flat tissues.

Thick gum tissue isn’t superior to thin, or vice-versa. In fact, each type is susceptible to certain types of diseases or adverse conditions.

Thin tissues are more susceptible to the occurrence of receding gums. Caused mainly by periodontal disease and toothbrush abrasion, the gum tissue recedes and exposes more of the unprotected tooth surface that should be below the gum line. This increases the risk of decay and tooth loss. Patients with thick tissue, on the other hand, have a higher risk of developing a condition known as “pocketing.” As the thicker gum tissue becomes inflamed from dental plaque, it loses its attachment to the teeth and forms a small pocket. The end result is possible bone and tooth loss.

There’s not much you can do about which type of gum tissue you have, for which you can thank (or blame!) your ancestors. But there’s something you can do to reduce your risk of periodontal disease. First and foremost, you should practice good daily hygiene, brushing with a soft-bristled tooth brush and gentle flossing. It’s also important to maintain regular cleanings and checkups in our office; not only will this ensure complete plaque and tartar removal, but gives us a better chance to detect either receding gums or pocketing early. Earlier detection can mean better treatment outcomes — and a saved smile.
